Your First Visit
So you have decided we are the office for you! We cannot wait to meet you!
Be sure to visit our forms page as well! We will need you to fill out the listed forms and send them to our office before your appointment. This allows us to review your medical history, input all information and preauthorize with your benefit company so we are ready for your first visit.
What you can expect
When we see you for a new patient exam you can expect a series of Xrays and about a 45 minute appointment to meet and have one of our doctors do a comprehensive exam. At this time it will give us the opportunity to get to know you and go over your dental history, concerns, and future treatment. Normally, a second visit will be scheduled in order to clean your teeth.
Cancellation Policy
Appointments are scheduled specifically for you! Our commitment to you is that we will make every effort to book an appointment that best suits your schedule.
We require 2 business days notice if you wish to change or cancel your upcoming appointment.
Payment
We will submit claims to your benefit plan on your behalf (please see Office Policies) and the remaining balance is due at the end of each appointment. We accept the following:
